This dissertation by Joseph Iskendiarso Sigit focuses on the development and application of a new gas chromatography-mass spectrometry (GC-MS) method for determining total plasma homocysteine and related amino acids. Using stable isotope dilution analysis, the method enables the quantification of homocysteine, methionine, cysteine, and cystathionine. Its analytical performance is evaluated through comparisons with established HPLC and immunoassay techniques.

The research also investigates homocysteine metabolism in patients with end-stage renal disease undergoing chronic dialysis. Particular attention is given to the effects of folic acid and leucovorin supplementation on elevated plasma homocysteine concentrations. The findings demonstrate that both treatments can reduce homocysteine levels, although normalization is not achieved in many patients. The study contributes to clinical homocysteine analysis and provides insights into amino acid metabolism in patients with renal insufficiency.
