The study was carried out in the Department of Pathology at RWTH Aachen (Head: Prof. Dr. Ch. Mittermayer).
In this work, experimentally induced animal and human transitional cell carcinomas of the urinary bladder were examined morphologically and morphometrically.
Using morphometric data, the relationships between the experimental model and the spontaneously occurring human carcinoma were examined.
The prognostic relevance of grading human bladder carcinoma according to the WHO classification was demonstrated in a retrospective analysis of approximately 2000 primary bladder carcinomas, whose course was documented for up to 7 years.
From DNA cytophotometric and karyometric measurements on 150 tumours, stepwise discriminant analysis was used to determine, for both methods, the descriptive measures that lead to optimal separation of normal findings and the 3 grades of differentiation of bladder carcinoma. In the form presented, these measures permit tumour classification free of subjectively determined inter- and intra-individual differences. The same morphometric measurement methods were also used to grade carcinoma in situ of the urinary bladder into three categories.
In the experimental section, the morphological diversity of bladder carcinoma induced in the rat and of its precursor lesions was differentiated and objectified by DNA cytophotometry and karyometry.
The mild to severe dysplasias occurring over the time course of tumour induction were examined comparatively by light microscopy as well as by scanning and transmission electron microscopy, and the findings obtained were correlated with the measurement data.
Through objective morphometric description, both the experiment and the morphological analysis of human bladder carcinoma are given generally valid significance.
